Eligibility
- To be eligible to participate in athletics at Discovery, students must complete a physical and parental consent annually. These forms are available at the front desk and on the school website. These must be completed before the first day of tryouts, practice or voluntary conditioning. Students must also have at LEAST a cumulative GPA of 2.0 to be eligible to play. Some coaches might have additional academic requirements to participate.

Athletes and Parents,
Please click on the link below for the DHS Athletics Participation Requirements. All parents/guardians of Discovery High School athletes must register with Aktivate. Click on the link below for instructions. The required Consent and Release form (EL3) will now be signed electronically on Aktivate. The completed (EL2) Physical form will no longer be turned in to the school, it will be uploaded to Aktivate.
Please remember that ALL athletes must submit a SPORTS physical on the appropriate physical form. The form can be dowloaded from the link below and can also be downloaded from Aktivate. We can NOT accept any other physical forms. The appropriate form is the EL2 (Physical). Please remember that high school physicals are different and are only valid for the current school year.
